Please note when booking your New Patient Appointment...
Contact Details: The full address and contact phone number of NRQi Studio will be included on your confirmation booking email. Only a general location is provided on the website.
Intake Form and Patient Consent: In order to have plenty of actual treatment time, you will need to fill out the "Intake Form" when you book your first appointment. This form will be available in your "Patient Portal" when you register and login to our booking page. This form provides your practitioner with;
- Your detailed health history.
- Your agreement with the terms and conditions of the NRQi Studio's Privacy Policy, Cancellation Policy, Informed Voluntary Consent Policy and Patient Age Requirement Policy.
If you have
not completed the "Intake Form" before your appointment, then part of your treatment time will have to be used to fill out the form, as you cannot be treated without providing formal written consent and a medical history. This form can take up to 20 minutes to complete, depending upon the detail of your health history. An example of the Patient Portal booking site is below. The
